Reducing readmissions in Oregon and beyond

Oregon’s No Place Like Home Campaign engaged hospitals, rehabilitation and skilled nursing facilities, hospices, home health agencies, community pharmacies, medical practices, community-based organizations, and other care providers in a collaborative statewide effort to reduce avoidable hospital readmissions that occur within 30 days of hospital discharge.

Acumentra Health, the primary supporting partner for No Place Like Home—Oregon, began community-based work to reduce readmissions in August 2011.  We assisted several Oregon communities, including the Portland metropolitan area, Lane County, Cascades West Council of Governments, and Southern Oregon. No Place Like Home enlarged the scope of this community-based work, expanding access to tools, materials, and learning and action networks for providers in all settings. Acumentra Health’s “readmissions scorecard” data reports helped providers and communities visualize the issue and identify areas of focus for systems change.

In August 2014, Acumentra Health began a new partnership as part of HealthInsight's four-state Quality Innovation Network-Quality Improvement Organization (QIN-QIO). Between 2014 and 2019, HealthInsight QIN-QIO will support communities in Nevada, New Mexico, Utah and Oregon in enhancing care coordination, with Acumentra Health coordinating activities for Oregon.
